Evaluation of carotid plaque standardized wall index(NWI)by three-dimensional ultrasound plaque quantitative analysis and its correlation with the coronary SYNTAX score

Abstract:Objective To evaluate carotid plaque normalized wall index(NWI)by real-time three-dimensional quantitative ultrasound and to analyze its correlation with the coronary SYNTAX score.Methods From January 2018 to June 2019,331 patients who underwent coronary angiography at the Department of Cardiology,Seventh Medical Center of PLA General Hospital(301 Hospital),were enrolled.All patients underwent three-dimensional carotid ultrasound examination,and NWI was obtained by three-dimensional quantitative ultrasound analysis of plaques.According to angiographically calculated SYNTAX score,patients were divided into four groups:control(n=82,score=0),mild(n=161,0<score≤22),moderate(n=52,23≤score≤32),and severe(n=36,score≥33).NWI values were compared among groups,and the correlation between NWI and SYNTAX score was analyzed.Results NWI values differed significantly among the control,mild,moderate,and severe groups(0.33±0.06 vs.0.36±0.07 vs.0.39±0.06 vs.0.47±0.12,P<0.001).In 223 patients with carotid plaque(mild,moderate,and severe groups),NWI correlated with SYNTAX score(r=0.488,P=0.001).Subgroup correlation coefficients were r=0.072(P=0.403)in the mild group,r=0.506(P=0.001)in the moderate group,and r=0.392(P=0.018)in the severe group.Conclusion Carotid plaque NWI measured by three-dimensional quantitative ultrasound correlates significantly with coronary SYNTAX score,suggesting that NWI may serve as a potential indicator for evaluating moderate to severe coronary atherosclerosis.
