A Case of CRT-D Implantation with Misdischarge in a Patient with Heart Failure Complicated by Atrial Fibrillation
Abstract:Atrial fibrillation and heart failure are two formidable challenges in cardiovascular medicine in the 21st century. These two diseases interact and influence each other, meaning that one can lead to the development or exacerbation of the other, ultimately causing a synergistic negative impact on cardiovascular health and the quality of life of patients. Although multiple studies have shown that atrial fibrillation ablation can improve cardiac function and prognosis in patients with atrial fibrillation and heart failure, not all patients are suitable. In this case, the patient had long-standing persistent atrial fibrillation combined with complete left bundle branch block, left atrial diameter (LA) of 5.0 cm, and left ventricular ejection fraction (LVEF) of 26%. The success rate of catheter ablation was low, and sinus rhythm was difficult to maintain after surgery, with unsatisfactory improvement in heart failure. Therefore, CRT-D implantation was chosen to improve heart failure. After surgery, the QRS complex was significantly shortened, and pharmacological treatment was used to control the ventricular rate.
