Abstract:Objective To compare the influence of ticagrelor in different doses on long-term prognosis in patients with acute coronary syndrome(ACS)complicated by hypertension. Methods ACS patients(n=125)were chosen from the First Affiliated Hospital of Xinjiang Medical University from Jan.2021 to Jan.2023.All patients were given antiplatelet therapy after percutaneous coronary intervention(PCI),and then divided into observation group(n=63)and control group(n=62)by using lottery method. The observation group was treated with ticagrelor in 180 mg loading dose, followed by sequential dose of 60 mg/time.The control group was treated with ticagrelor in 180 mg loading dose, followed by sequential dose of 90 mg/time.The patients were followed up for 1 y,and clinical efficacy was compared between 2 groups. Results The baseline data was comparable between 2 groups before treatment(P>0.05).The levels of prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT)and fibrinogen(FIB)increased in 2 groups after treatment. The time of TT,PT and APTT is significantly prolonged, and FIB is decreased. In observation group than those in control group(P<0.05).After treatment, cardiac index(CI)and left ventricular ejection fraction(LVEF)increased, left ventricular end-diastolic diameter(LVEDD),mean arterial pressure(MAP)and endothelin-1(ET-1)decreased,and nitric oxide(NO)increased in 2 groups. The incidence of adverse cardiovascular events(4.76%vs.9.68%)and adverse drug reactions(1.59%vs.12.9%)were lower in observation group than those in control group(P<0.05).Conclusion Ticagrelor in different doses can improve vascular endothelial function and recovery of cardiac function, and in lower sequential dose, ticagrelor can significantly reduce risks of adverse drug reactions and adverse cardiovascular events with higher safety in patients with ACS complicated by hypertension.
