Exploring the ameliorative effects of Guanxinning tablets on clozapine-induced cardiovascular diseases based on the"gut-heart axis"theory

Abstract：Objective To investigate the ameliorative effect and mechanism of Guanxinning tablets on cardiovascular diseases caused by clozapine based on the"gut-heart axis"theory.Methods Sixty schizophrenia patients who have been taking clozapine for more than 3 years with abnormal electrocardiogram manifestations and were admitted to Jiande Fourth People's Hospital from January 2022 to January 2023 were selected and randomly divided into an experimental group and a control group,with 30 cases in each group.The experimental group was treated with Guanxinning tablets(3 times a day,4 tablets each time)for 12 weeks in addition to clozapine.All patients were followed up for blood lipids,blood glucose,myocardial enzyme levels,N-terminal pro-brain natriuretic peptide(NT-proBNP),and 12-lead electrocardiogram changes before and after treatment.After 12 weeks treatment,fresh feces were collected to detect intestinal flora,and blood samples were collected for plasma metabolomics analysis.Results Compared with the control group,the levels of low-density lipoprotein in the patients of the experimental group were significantly decreased at 4 weeks,8 weeks and 12 weeks after treatment(P＜0.05).The frequency of arrhythmia in patients was significantly reduced 12 weeks after treatment(P＜0.05).There was no significant change in myocardial enzyme-related indicators or NT-proBNP levels in either group before and after treatment(P＞0.05).In the experimental group,the relative abundance of Firmicutes was reduced,and the relative abundance of Bacteroidetes was increased(P＜0.01);at the genus level,the relative abundance of Roseburia and Parabacteroides was increased(P＜0.001).In terms of metabolomics,Guanxinning tablets can significantly increase the level of butyric acid in the patient's blood(P＜0.001).Conclusion Guanxinning tablets can effectively improve the electrocardiogram performance of patients who have been using clozapine for a long time,and this effect may be achieved through the"gut heart axis"to improve the structure of gut microbiota and its metabolites.
