Predictive value of computed tomography-fractional flow reserve to major adverse cardiovascular events in young and middle-aged patients with moderate coronary stenosis

Abstract:Objective To combine technique of computed tomography-fractional flow reserve(CT-FFR)and risk factor indexes of major adverse cardiovascular events(MACE),and study the predictive value of CT-FFR to MACE.Methods The young and middle-aged patients(n=517)with moderate coronary stenosis diagnosed by coronary computed tomography angiography(CCTA)were chosen,and CT-FFR value was calculated in Chinese PLA 305th Hospital from Jan.2017 to Jan.2021.All patients were divided into MACE group and non-MACE group.The value of CT-FFR,severity of coronary stenosis and main cardiovascular risk factors were predicted by using Cox risk regression model in the patients.The area under curve(AUC)of receiver operating characteristic(ROC)curve was calculated to review the predictive value of AUC to MACE.Results The results of single-factor analysis showed that differences in homocysteine(Hcy),triglyceride-glucose index(TyG),severity of coronary stenosis and CT-FFR value had statistical significance(P<0.05),and they were independent risk factors to MACE.The results of Cox regression analysis showed that TyG≥8.83,Hcy≥ 16.03 μmol/L,severity of coronary stenosis ≥60%and CT-FFR value ≤0.85 were independent risk factors to MACE(P<0.05).The predictive model based on CT-FFR+severity of coronary stenosis+clinical cardiovascular risk factors(AUC=0.979,95%CI:0.962~0.997)were superior to that based on CT-FFR model and that based on CT-FFR+severity of coronary stenosis(P<0.05).Conclusion The predictive model based on CT-FFR+severity of coronary stenosis+clinical cardiovascular risk factors has a higher diagnostic efficacy,which can provide reference for timely clinical intervention and prognosis improvement in young and middle-aged patients with moderate coronary stenosis.
