Network meta-analysis of non-pharmaceutical interventions on sleep disorders in elderly coronary heart disease patients

Abstract:Objective This study employed a network meta-analysis to systematically evaluate the effects of various non-pharmaceutical interventions on improving sleep disorders in elderly patients with coronary heart disease(CHD).Methods A comprehensive literature search was conducted in multiple domestic and international databases,including China National Knowledge Infrastructure(CNKI),WanFang Database,VIP Database,and PubMed,to collect randomized controlled trials(RCTs)published up to June 1,2024,regarding the effects of non-pharmaceutical interventions on sleep disorders in elderly CHD patients.Both Chinese and English search strategies were applied.Relevant studies were selected based on predefined inclusion and exclusion criteria,and data were extracted accordingly.The risk of bias was assessed using RevMan 5.3,and network meta-analysis was performed using the network package and Stata 16.0.Results A total of 11 RCTs involving 912 patients and 11 types of non-pharmaceutical interventions were included.The results showed that sleep intervention combined with targeted nursing,medicinal moxibustion on the navel,targeted nursing,Shuxin Yangqi exercises,copper scraping therapy on ears,ear acupressure,systematic insomnia intervention,sleep psychological and behavioral intervention,sleep intervention,and rational-emotive behavioral intervention were all significantly more effective than routine care,with statistically significant differences.The interventions were ranked from most to least effective as follows:sleep intervention combined with targeted nursing>rational-emotive behavioral intervention>ear acupressure>targeted nursing>copper scraping therapy on ears>medicinal moxibustion on the navel>Shuxin Yangqi exercises>sleep psychological and behavioral intervention>sleep intervention>systematic insomnia intervention>routine care.Among these,sleep intervention combined with targeted nursing showed the most significant improvement in PSQI scores(SMD=-2.17,95%CI:-3.49~-0.85).Conclusion Current evidence indicates that among the 11 non-pharmaceutical interventions,sleep intervention combined with targeted nursing is the most effective treatment for improving sleep disorders in elderly CHD patients.
