Meta-analysis of the effectiveness of patent foramen ovale closure versus medical therapy for cryptogenic stroke

Abstract:Objective To systematically evaluate the effectiveness and safety of patent foramen ovale(PFO)closure versus medical treatment for cryptogenic stroke(CS).Methods Using database search tools,we collected randomized controlled trial data comparing PFO closure with medical therapy for cryptogenic stroke,and subsequently conducted a Meta-analysis for a comprehensive evaluation.Results A total of 8 articles were included,involving 4823 patients.Compared with medical therapy,the incidence of recurrent stroke after percutaneous closure of PFO was reduced(OR:0.49,95%CI:0.35~0.68;heterogeneity P=0.38,I2=7%).There were no statistically significant differences in the risk of transient ischemic attack(TIA,OR:0.78,95%CI:0.49~1.24;heterogeneity P=0.52,I2=0%),embolism(OR:2.16,95%CI:0.6~7.76;heterogeneity P=0.14,I2=54%),atrial fibrillation(AF,OR:4.57,95%CI:0.99~21.17;heterogeneity P=0.08,I2=60%),and mortality(OR:0.63,95%CI:0.26~1.53;heterogeneity P=0.7,I2=0%)between the two treatment options.Conclusion PFO closure is superior to medical treatment in preventing recurrent stroke and does not increase the risk of embolism,atrial fibrillation,or mortality.
