Correlation analysis between CHA2DS2 VASc score and left atrial and left atrial appendage indicators in patients with non-valvular atrial fibrillation

Abstract:Objective The purpose of this study was to better evaluate the risk of thromboembolism in patients with non-valvular atrial fibrillation(AF)by exploring the correlation between left atrial(LA)and left atrial appendage(LAA)indicators and CHA2DS2-VASc score.Methods We enrolled 50 non-valvular AF patients attending at our hospital.And they were divided into a high-risk group for thromboembolism and a low-medium risk group according to the CHA2DS2-VASc score..Their Hcy,hs-cTnT,NT-proBNP were collected.Their left atrial volume index(LAVI),left atrial emptying fraction(LAEF),left atrial strain and strain rate during reservoir phase(LASr and PLASRr),Left atrial stiffness index(LASI)were assessed by echocardiography.Their left atrial appendage(LAA)orifice area,perimeter,long diameter,short diameter,and LAA deep diameter,LAA morphology,LAA volume index(LAAVI)were assessed by electrocardiogram-gated computed tomography angiography.Spearman correlation analysis was used to explore the correlation between CHA2DS2-VASc score and NT-proBNP,LASr,LASI,LAA orifice area.Results The hs-cTnT、NT-proBNP in the high risk group were higher than those in the low-medium risk group(P<0.05),the difference of Hcy were very little(P>0.05);The LAVImax and LAVImin,LASI in the high risk group were higher than those in the low-medium risk group(P<0.05).The LAEF,LASr,PLASRr in the high risk group were lower than those in the low-medium risk group(P<0.05).The LAA orifice area in the high risk group was smaller than those in the low-medium risk group(P<0.05);the difference of LAA perimeter,long diameter,short diameter,and LAA deep diameter,LAA morphology,LAAVI were very little between two groups(P>0.05).In spearman correlation analysis,CHA2DS2-VASc score was negatively correlated with LASr(r=-0.628,P<0.05)and positively correlated with NT-proBNP,LASI,LAA orifice area(r=0.317,r=0.544,r=0.374,P>0.05).Conclusion A novel evaluation method that combines LASr,LASI,LAA orifice area,NT-proBNP with CHA2DS2-VASc score performed better than the presently used clinically based risk score in AF.
