Influence of different anesthesia methods on therapeutic effect of radiofrequency ablation in patients with atrial fibrillation

Abstract:Objective To compare the influence of different anesthesia methods on perioperative and postoperative therapeutic effects of radiofrequency ablation(RFA)in patients with atrial fibrillation(AF).Methods The patients with non-valvular atrial fibrillation(NVAF,n=207)were retrospectively analyzed in School of Civil Aviation Clinical Medicine of Peking University from July 2018 to Sept.2022.The patients were divided,according to anesthesia methods,into general anesthesia group(GA group)and group of local infiltration anesthesia combined with intravenous analgesia(LA group).The patients in 2 groups were divided,according to gender,into subgroups.The total operation time,time of digital subtraction angiography(DSA),left atrial procedure time,total RFA time and incidence of atrial arrhythmias within 52 weeks postoperatively were compared between 2 groups.The differences in relevant indexes were analyzed between 2 anesthesia methods in different groups and subgroups during RFA.Results The differences in the total operation time and DSA time had no statistical significance between GA group and LA group(P>0.05).GA group showed significantly better outcomes than LA group in terms of left atrial procedure time,total RFA time and the incidence of atrial arrhythmias within 52 weeks postoperatively.The results of subgroup analysis showed that male subgroup of GA group had significantly better outcomes thanmale subgroup LA group in terms of left atrial procedure time,total RFA time and the incidence of atrial arrhythmias within 52 weeks postoperatively(P<0.05).The female subgroup of GA group showed significantly better outcomes than female subgroup of LA group in terms of left atrial procedure time and total RFA time(P<0.05),but there was no statistical difference in incidence of atrial arrhythmias within 52 weeks postoperatively between 2 female subgroups(P>0.05).Conclusion Both general anesthesia and local infiltration anesthesia combined with intravenous analgesia can provide safety assurance for RFA in AF patients.Some clinical indicators in GA group were superior to those in LA group,and male patients benefited more from RFA performed under general anesthesia.
