Predictive value of MELD-XI score for no-reflow phenomenon and short-term mortality in patients with ST-segment elevation myocardial infarction treated with PCI

Abstract:Objective To analyze the predictive value of the End Stage Liver Disease XI(MELD-XI)scoring model for no-reflow phenomenon and 30-day all-cause mortality in patients with ST-segment elevation myocardial infarction(STEMI)undergoing primary percutaneous coronary intervention(PCI).Method A retrospective analysis was conducted on the 557 STEMI patients treated with PCI in our hospital from January 2020 to December 2023.According to the blood flow classification of myocardial infarction thrombolysis(TIMI),patients were divided into the normal reflux group(n=438)and the no-reflow group(n=119).Clinical data were collected from two groups of patients and the MELD-XI score was calculated.Multivariate Logistic regression analysis was used to determine potential risk factors for no-reflow phenomenon.The receiver operating characteristic curve(ROC)and area under the curve(AUC)were plotted to evaluate the predictive value of MELD-XI score for no-reflow phenomenon and 30-day all-cause mortality.Results Multivariate logistic regression analysis showed that diabetes(OR=2.173,P<0.001),history of peripheral vascular disease(OR=3.447,P=0.001),elevated hs-CRP(OR=1.275,P=0.008),and elevated MELD-XI score(OR=1.347,P=0.001)were independent risk factors for no-reflow phenomenon.The AUC of MELD-XI score for predicting no-reflow phenomenon was 0.853,with cut-off point at 9.47,sensitivity and specificity were 81.93%,78.67%.The AUC of MELD-XI score for predicting 30-day all-cause mortality was 0.772.Conclusion The MELD-XI score can be used for predicting no-reflow phenomenon and 30-day all-cause mortality in STEMI patients undergoing PCI treatment.The MELD-XI score is a potential risk stratification tool for STEMI patients and deserves further exploration and research.
