Relationship between D-dimer to high sensitivity cardiac troponin T ratio and malignant ventricular ar-rhythmia in patients with acute myocardial infarction

Abstract:Objective To investigate the relationship between D-dimer(D-D)to high sensitivity cardiac troponin T(hs-cTnI)ratio(Rdc)and malignant ventricular arrhythmia(MVA)in patients with acute myocardial infarction(AMI).Methods The clinical materials were collected from AMI patients(n=236)in Qianjiang Hospital affiliated to Chongqing University from Jan.2023 to Dec.2023.According to whether MVA occurred within 24 h after admission,the patients were divided into non-MVA group(n=167)and MVA group(n=69).The levels of D-D and hs-cTnI were detected by using chemiluminescence immunoassay at admission time,and Rdc was calculated.The value of Rdc to MVA was reviewed by using ROC curve in AMI patients.The influence factors for MVA occurrence in AMI patients were analyzed by using multi-factor Logistic regression analysis.Results Among 236 AMI patients 24 h after admission,there were 69(29.24%)complicated by MVA,and Rdc value was significantly higher in MVA group than that in non-MVA group(P<0.05).The results of ROC curve analysis showed that,in reviewing MVA occurrence in AMI patients,AUC of D-D was 0.795(95%CI:0.750~0.840),AUC of hs-cTnI was 0.703(95%CI:0.658~0.753),and AUC of Rdc was 0.908(95%CI:0.858~0.957).The differences in age,gender,body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP)and ventricular rate at admission time,percentage of left ventricular ejection fraction(LVEF)<50%,lesion types,and percentages of smoking and drinking history had no statistical significance between 2 groups(P>0.05).The percentages of Killip grades from Ⅲ to Ⅳ and inferior and posterior wall infarction were higher in MVA group than those in non-MVA group(P<0.05).The results of multi-factor regression analysis showed that Killip grades from Ⅲ to Ⅳ(OR=2.121,95%CI:1.005~4.476),inferior and posterior wall infarction(OR=2.784,95%CI:1.461~5.306)and Rdc≥76.63(OR=4.802,95%CI:1.999~11.532)were influence factors for in AMI patients complicated by MVA(P<0.05).Conclusion Rdc is closely correlated to MVA occurrence in AMI patients,and can be used as an auxiliary biomarker to review MVA occurrence in AMI patients.
