Differences in burdens of common chronic cardiovascular and non-cardiovascular diseases in elderly pa-tients with chronic heart failure and different ejection fractions:investigation and analysis

Abstract:Objective To investigate the burdens of common chronic cardiovascular and non-cardiovascular diseases in elderly patients with chronic heart failure(CHF)and different ejection fractions.Methods The data were retrospectively analyzed in CHF patients(n=1096)in Geriatric Multidisciplinary Diagnosis and Treatment Center of Hainan Chengmei Hospital from Jan.2020 to Dec.2022.According to examination results of left ventricular ejection fraction(LVEF),the patients were divided into group of heart failure with preserved ejection fraction(HFpEF,LVEF≥50.0%,n=644)and group of heart failure with reduced ejection fraction(HFrEF,LVEF<50.0%,n=452).The medical records were reviewed,and indexes of common cardiovascular diseases,non-cardiovascular diseases,treatment burden were statistically analyzed in 2 groups.Results The difference in the number of cardiovascular diseases had no statistical significance between HFrEF group and HFpEF group(P>0.05).The percentages of coronary heart disease(CHD),cardiomyopathy or myocarditis were higher(P<0.05),and those of hypertension,atrial fibrillation(AF)or atrial flutter(AFL)were lower(P<0.05)in HFrEF group than those in HFpEF group.The number of chronic non-cardiovascular diseases was lower in HFrEF group than that in HFpEF group(P<0.05).The percentage of dyslipidemia was higher(P<0.05),and those of anemia,cerebrovascular disease and chronic obstructive pulmonary disease(COPD)were lower(P<0.05)in HFrEF group than those in HFpEF group.The percentages of patients with 0~2 or 3~4 chronic diseases were higher(P<0.05),and percentage of patients with more than 5 chronic diseases was lower(P<0.05)in HFrEF group that those in HFpEF group.The medication burden score,visit score and total score of chronic disease co-existing treatment burden were lower in HFrEF group than those in HFpEF group(P<0.05).There was no significant difference in lifestyle scores between HFrEF group and HFpEF group(P>0.05).The difference in lifestyle scores had no statistical significance between HFrEF group and HFpEF group(P>0.05).Conclusion There are significant differences in the spectrum and burden of cardiovascular and non-cardiovascular diseases in elderly CHF patients with different levels of LVEF.The treatment burden is higher in HFpEF patients than that in HFrEF patients.For elderly CHF patients,comprehensive prevention and treatment of diseases,including CHD,hypertension,AF or AFL and anemia,should be emphasized.
