Influence of continuous improvement of chest pain center on reperfusion and prognosis in patients with acute ST-segment elevation myocardial infarction

Abstract:Objective To investigate the influence of continuous improvement of chest pain center on reperfusion and prognosis in patients with acute ST-segment elevation myocardial infarction(STEMI).Methods The patients with acute STEMI(n=176)were selected from People's Hospital of Lu'an City of Anhui Province from Oct.2019 to Sept.2021,and divided,according to hospitalization time,into control group(from Oct.2019 to Sept 2020)and study group(from Oct.2020 to Sept.2021,each n=88).The control group was treated in routine emergency chest pain center,and study group was additionally treated in chest pain center with continuous improvement.The various rescue times and surgical conditions[time from first medical contact time(FMC)to consent for PCI,catheterization lab activation time,door-to-balloon(D-to-B)time,time from FMC to PCI completion],improvements in treatment process(proportion of remote ECG transmission,proportion bypassing CCU and emergency department),length of hospital stay,hospitalization costs and prognosis were compared between 2 groups.Results Compared with control group,study group demonstrated significantly better performance in the completion time of the first ECG,troponin testing,diagnosis confirmation,emergency department stay duration,and FMC-to-PCI time(P<0.05),and had higher rates of remote ECG transmission(73.86%vs.50.00%)and bypassing CCU/emergency(67.05%vs.51.14%,P<0.05),and shorter hospital stays,lower medical costs and lower incidence rates of malignant arrhythmia(3.41%vs.11.36%)and heart failure(4.55%vs.13.64%,P<0.05).Conclusion A continuous improvement plan for service quality of chest pain center can effectively increase the percentages of remote ECG transmissions,bypassing CCU and emergency room visits,reduce rescue time,operation time and hospitalization time,improve prognosis,and decrease treatment costs in STEMI patients.
