Characteristic predictors of dilated cardiomyopathy complicated by ventricular arrhythmias

Abstract:Objective To identify the clinical characteristics in patients with dilated cardiomyopathy(DCM)complicated by ventricular arrhythmias(VA),and analyze the influence factors for VA occurrence in DCM patients.Methods The patients with DCM complicated by VA were selected into DCM-VA group(n=78),and patients with only DCM were selected into DCM group(n=105)from the Seventh People's Hospital of Zhengzhou City from Jan.2021 to Dec.2023.The demographic data,clinical biochemical indexes,and results of routine ECG and echocardiography were collected and differences were analyzed between 2 groups.The independent variables were screened by using Lasso regression,and influence factors for VA occurrence were analyzed by using binary multi-factor Logistic regression in DCM patients.The potential diagnostic value of influence factors was analyzed by using ROC curve analysis.Results The results of single-factor analysis showed that age increased,level of N-terminal pro-brain natriuretic peptide(NT-proBNP)increased,QRS duration and Q-T interval prolonged,left atrial diameter(LAD)and left ventricular end-diastolic diameter(LVEDd)increased,and left ventricular ejection fraction(LVEF)decreased in DCM-VA group compared with DCM group.The results of Logistic regression analysis showed that aging,increased NT-proBNP,LAD and LVEDd were independent predictive factors for DCM complicated by VA(P<0.05).The results of ROC curve analysis showed that age(AUC=0.802),LAD(AUC=0.907),LVEDd(AUC=0.864)and combination of these 3 indexes(AUC=0.936)had higher diagnostic efficacy in predicting DCM complicated by VA.Conclusion Advised age and LAD and LVEDd increases may be predictive factors for DCM complicated by VA,and have certain predictive value to VA occurrence in DCM patients in clinical diagnosis.
