Factors related to renal insufficiency in patients with chronic heart failure complicated by type 2 diabetes mellitus

Abstract：Objective To analyze the risk factors related to renal insufficiency(RI)and their predictive value to RI in patients with chronic heart failure(CHF)complicated by type 2 diabetes mellitus(T2DM).Methods The patients with CHF complicated by T2DM were chosen from the First People's Hospital of Wuhu City from Jan.2021 to Dec.2023.All patients were divided,according to they had not had RI,into RI group and non-RI group.The differential variables were compared by using single-factor analysis between RI group and non-RI group.The potential relationship between differential variables and RI occurrence was analyzed by using Logistic regression analysis.The predictive value of potential variables to RI occurrence was analyzed by using ROC curve in patients with CHF complicated by T2DM.Results There were totally 267 patients with CHF complicated by T2DM included in this study(79 in RI group and 188 in non-RI group).Age[95％CI:1.106(1.055～1.159)],T2DM course[95％CI:1.558(1.312～1.852)],valvular heart disease[95％CI:2.126(1.112～8.066)]and uric acid[UA,95％CI:1.012(1.008～1.016)]were independent risk factors for RI occurrence.Hemoglobin[Hb,95％CI:0.935(0.913～0.958)]was an independent protective factor for RI occurrence.Age[0.66(0.611～0.708)],T2DM course[0.723(0.676～0.769)],valvular heart disease[0.573(0.519～0.628)],Hb[0.705(0.658～0.751)]and UA[0.735(0.688～0.782)],especially UA,had some predictive value to RI occurrence in patients with CHF complicated by T2DM.In the valuation of combined above indexes in prediction efficacy for RI,AUC was 0.938(0.917～0.960).Conclusion Age,T2DM course,valvular heart disease and UA are useful indicators for RI occurrence,and combination of them has the highest predictive efficacy in patients with CHF complicated by T2DM.
