Predictive value of uric acid/albumin ratio combined with prognostic nutrition index to contrast-induced acute kidney injury in patients with ST-segment elevation myocardial infarction after percutaneous coronary intervention

Abstract:Objective To investigate the predictive value of uric acid/albumin ratio(UAR)combined with prognostic nutrition index(PNI)to contrast-induced acute kidney injury(CI-AKI)in patients with ST-segment elevation myocardial infarction(STEMI)after percutaneous coronary intervention(PCI).Methods STEMI patients undergone PCI(n=1085)were retrospectively chosen into training group from Department of Cardiovascular Medicine in Affiliated Hospital of Xuzhou Medical University from Feb.2019 to May 2023.The elderly STEMI patients undergone PCI(n=370)were chosen into validation group from East Branch of Affiliated Hospital of Xuzhou Medical University from June 2022 to May 2023.The training group was divided,according to CI-AKI occurrence after PCI,into CI-AKI subgroup(n=94)and non-CI-AKI subgroup(n=991).The influence factors for CI-AKI occurrence were analyzed by using single-factor and multi-factor Logistic regression analysis.The predictive value of above factors to CI-AKI was reviewed by using ROC curve.All patients were further grouped according to the results of restricted cubic spline analysis,and the relationship among different groups and CI-AKI risk was compared.Results CI-AKI occurred in 94 patients in this study,and CI-AKI incidence rate was 8.66%.The results of multi-factor Logistic regression analysis showed that age,female,left ventricular ejection fraction(LVEF),diuretics administration,UAR and PNI were independent risk factors for CI-AKI occurrence in STEMI patients after PCI(P<0.05).The results of ROC curve analysis showed that,in predicting CI-AKI occurrence,AUC of UAR combined with PNI was 0.801(95%CI:0.763~0.839),sensitivity was 83.0%and specificity was 63.8%,which was superior to those of UAR alone or PNI alone.The results of restricted cubic spline analysis showed that UAR>8.0 was positively correlated to CI-AKI occurrence,and PNI<37.5 was negatively correlated to CI-AKI occurrence.Conclusion UAR combined with PNI can predict the possibility of CI-AKI occurrence and closely correlated to CI-AKI risk in STEMI patients after PCI.
