Application sequence of drug-coated balloon in coronary bifurcation lesions

Abstract：Objective To analyze the influence of application sequence of drug-coated balloon(DCB)for coronary bifurcation lesions on operation effect and clinical prognosis.Methods The patients with coronary heart disease(CHD)were retrospectively analyzed in People's Hospital of Weifang City and its branch hosipital from June 2020 to June 2022.The coronary bifurcation lesions were detected by using coronary angiography(CAG),which were divided into Medina(1,0,1),Medina(1,1,1)and Medina(0,1,1).The therapy was DCB in branch vessel+drug stent implantation in main vessel.According to DCB application sequence,the patients were divided into Pre-DCB group(DCB application before stent implantation)and post-DCB group(DCB application after stent implantation).The baseline characteristics were observed in 2 groups,including bifurcation type,bifurcation position,stent length,stent release pressure,DCB length,DCB release pressure and kiss dilation status.The patientswere given CAG re-examintion immediately after the operation and after 1 y,and minimal lumen diameter(MLD),lumen stenosis rate(DS%)and late lumen loss(LLL)were observec in 2 groups.The occurrences of major adverse cardiovascular events(MACE)and relapse of angina pectoris(AP)were observated in 2 groups during follow-up period.Results The pressure of DCB dilation was 6(6,8)atmin Pre DCB group,and 8(6,8)atm in Post DCB group(P=0.0172).There were 48 cases(72.7%)completed kiss dilation in Pre DCB group,and 52(96.3%)in Post-DCB group(P=0.0006).The results of CAG re-examintion immediately afte opereaion and after 1 y showed that MLD was larger(P＜0.01),and DS%was lower(P＜0.01)in Post-DCB group than those in Pre-DCB group,and LLL was more significant in Pre-DCB group(P＜0.01).The results of follow-up showed that occurrences of MACE and AP relapse had no statistical significance between 2 groups.Conclusion In treatment of coronary bifurcation lesions,both opertive plans,DCB applications before and after stent implantation,have higher clinical efficacy and good follow-up results.The therapy of stent implantation followed by DCB has some advantages in maintaining a larger branch opening area immediately after the operation and during follow-up observation.
