Analysis of mortality risk factors in CTEPH patients based on an intelligent healthcare data platform

Abstract:Objective To investigate mortality risk factors among patients with chronic thromboembolic pulmonary hypertension(CTEPH)using an intelligent healthcare data platform.Methods Patient data from the CTEPH healthcare database were collected from June 2021 to June 2023.Based on follow-up results,patients were divided into a survival group(n=94)and a mortality group(n=14).Baseline characteristics,clinical manifestations,and pulmonary embolism severity index(PESI)scores were compared between the two groups.Logistic regression analysis was performed to identify risk factors for mortality in CTEPH patients.Result A total of 108 patients were included,with 94 in the survival group and 14 in the mortality group.Compared with the survival group,the mortality group had a higher proportion of patients with New York Heart Association(NYHA)class Ⅳ heart function,higher levels of serum creatinine(SCR),blood urea nitrogen(BUN),and aspartate aminotransferase(AST),and lower levels of estimated glomerular filtration rate(eGFR),with all differences being statistically significant(P<0.05).The mortality group also had significantly higher proportions of patients experiencing symptoms such as dyspnea on exertion,chest tightness,syncope,peripheral edema,and cyanosis(P<0.05).Additionally,the proportion of patients in PESI class Ⅲ~Ⅴ was significantly higher in the mortality group than in the survival group(P<0.001).Logistic regression analysis showed that SCR and PESI were independent risk factors for mortality in CTEPH patients(P=0.015,P=0.028).Conclusion SCR and PESI are key independent predictive factors for mortality in CTEPH patients.Therefore,clinical management of CTEPH should emphasize monitoring SCR and the early prevention of pulmonary embolism(PE)to timely identify and intervene with high-risk patients,thereby improving patient outcomes.
