Correlation between combined circumferential pulmonary-vein ablation and recurrence of atrial fibrilla-tion:a Meta-analysis

Abstract:Objective To study the effectiveness of circumferential pulmonary-vein ablation(CPVA)combined with ablation at other positions in reducing recurrence of atrial fibrillation(AF)after ablation.Methods The databases of CNKI,WanFang Data,VIP,CBM,PubMed,EMbase and Cochrane Library were retrieved with computer to screen eligiblerandomized controlled trials(RCT).After screened literature,extracted data and reviewed bias risk of included studies by 2 researchers independently,a Meta-analysis was conducted by using Stata software.The difference in AF recurrence after ablation was summed up and compared among CPVA alone and CPVA combined with ablation strategies of left atrial roof(LR),mitral valve isthmus(MVI),left atrial posterior wall(LAPW)and superior vena cava(SVC).Results There were totally 16 RCT included.The results of Meta-analysis showed that CPVA alone and ablation strategies of CPVA combined with other positions had no significant difference in reducing AF recurrence(RR=1.00,95%CI:0.88~1.14,P>0.05),and was no correlated to AF types.Compared with CPVA alone,the recurrence rate of postoperative AF was not reduced further by ablation strategies of CPVA combined with LR,MVI,SVC and BOX.Conclusion The recurrence rate of postoperative AF is not reduced by ablation strategies of CPVAcombined with other positions compared with CPVA alone.
