Influence of blood pressure variability and heart rate variability on ventricular arrhythmias in patients with hypertension complicated by heart failure

Abstract：Objective To discuss the influence of blood pressure variability(BPV)and heart rate variability(HRV)on ventricular arrhythmias(VA)in patients with hypertension complicated by heart failure(HF).Methods The patients(n=164)with hypertension complicated by HF were retrospectively analyzed in Department of Cardiovascular Medicine in Special Medical Center of Chinese PLA Rocket Force from Mar.2021 to Feb.2023,and according to with or without VA,the patients were divided into VA group(n=76)and non-VA group(n=88).The general materials were analyzed,and HRV time domain indexes(SDNN,SDNN index,SDANN,RMSSD,PNN50),frequency domain indexes[low frequency band(LF),high frequency band(HF)and LF/HF ratio],and BPV indexes[24 h systolic standard deviation(SSD),24 h diastolic standard deviation(DSD),day systolic standard deviation(dSSD),day diastolic standard deviation(dDSD),night systolic standard deviation(nSSD),night diastolic standard deviation(nDSD),24 h systolic blood pressure coefficient of variation(sCV),and 24 h diastolic blood pressure coefficient of variation(dCV)]were detected in 2 groups.The influence factors of VA were analyzed by using multi-factor Logistic regression analysis.The predictive values of HRV and BPV to VA were analyzed by using ROC curve.Results The results of multi-factor Logistic regression analysis showed that SDNN,SDNN,SDANN,SSD,dSSD,nSSD and sCV were influence factors of VA(OR=0.861,0.825,0.761,2.630,2.630,3.555,2.603,2.840;95％CI:0.744～0.997,0.690～0.986,0.604～0.959,1.086～6.368,1.156～10.926,1.160～5.844,1.093～7.383;P=0.045,0.034,0.021,0.032,0.027,0.020,0.032)in patients with hypertension complicated by HF.The results of ROC curve analysis showed that SDNN,SDNN index,SDANN,SSD,dSSD,nSSD and sCV had some predictive values to VA occurred(AUC=0.784,0.824,0.834,0.816,0.816,0.866,0.891;95％CI:0.713～0.845,0.757～0.879,0.768～0.887,0.748～0.872,0.748～0.872,0.805～0.914,0.833～0.935).Conclusion BPV and HRV are influence factors of VA with some predictive values to VA in patients with hypertension complicated by HF.
