Comparison of clinical characteristics and preventionstrategiesof pulmonary embolism between young adults in plateau region and middle-aged and elderly individuals in plain region

Abstract:Objective To explore the difference of clinical characteristics and prevention of pulmonary embolism between plateau youth and plain elderly.Methods A total of 32 patients diagnosed with pulmonary embolism in the Department of Cardiology,General Hospital of Xinjiang Military Command from September 2019 to October 2021 were collected.According to age,they were divided into the young group(15 cases)and the middle-aged and elderly group(17 cases).In the young group,the disease started at high altitude and was diagnosed after referral.The middle-aged and elderly patients were the confirmed cases hospitalized in the same period.The differences in clinical characteristics,examination results and prognosis between the two groups were summarized and compared.Results Compared with the youth group,the elderly group had a shorter time from symptom onset to treatment[(6.71±7.23)vs.(21.87±23.36)days,P<0.05],the elderly group was older[(72.76±12.68)vs.(24.8±5.16)years,P<0.05],the proportion of chronic diseases(COPD,chronic bronchitis,asthma,pulmonary hypertension history,pulmonary heart disease,coronary heart disease,hypertension,malignant tumor,diabetes)was higher(P<0.05),and the differences were statistically significant;Compared with the youth group,the main pulmonary artery diameter of the elderly group was significantly wider[(32.76±3.70)vs.(28.81±4.50)mm,P<0.05],and the ECG manifestations of acute pulmonary embolism in the elderly group were higher than those in the youth group(58.8%vs.20.0%,P<0.05).The mortality rate of the elderly group was significantly higher than that of the young group(23.5%vs.0,P<0.05).Conclusion The high altitude environment leads to a significant prolongation of the treatment time for acute pulmonary embolism in young people.Therefore,attention should be paid to early identification and treatment.High-altitude pulmonary embolism is more likely to be complicated by hyperuricemia,so it is necessary to strengthen education and change bad lifestyles to improve abnormal metabolism.
