Influence of ShexiangBaoxin Pills on cardiovascular outcomes in patients with coronary heart disease complicated by diabetes mellitus:a systematic review and Meta-analysis

Abstract：Objective To compare the influence of ShexiangBaoxin Pills combined with standard treatment on cardiovascular outcomes in patients with coronary heart disease complicated by diabetes mellitus(CHD+DM).Methods The databases of PubMed,EMbase,The Cochrane Library,CNKI,WanFang and SinoMe were systematically retrieved for collecting the randomized controlled trials(RCT)of CHD+DM patients treated withShexiangBaoxin Pills from database establishment time to July 12,2023.The retrieving was in a manner of free word combined with subject term without publishing language restrictions.The primary outcome was major adverse cardiovascular events(MACE),and secondary outcomes included angina pectoris(AP),heart failure(HF)and target lesion revascularization(TLR).The quality of included RCT was reviewed by using Cochranebias risk tool and GRADE.Results There were totally 9 RCT with 1656 patients included.The results of systematicalanalysis showed that moderate quality evidenceindicated that ShexiangBaoxin Pills decreased MACE incidence compared with standard treatment(RR=0.38,95％CI:0.23～0.63,P＜0.001).When therapeutic course≥ 12,moderate and low quality evidences indicated that ShexiangBaoxin Pills significantly decreased incidence rates of AP(RR=0.62,95％CI:0.46～0.83,P=0.001),HF(RR=0.36,95％CI:0.14～0.89,P=0.03)and TLR(RR=0.47,95％CI:0.24～0.90,P=0.02).Conclusion The moderate quality evidence indicates that long-term administration of ShexiangBaoxin Pills can significantly decrease MACE incidence compared with standard treatment.Due to limited quality and quantity of the included RCT,the conclusion should beverifiedfurther by high-quality studieswith large sample and multi-center.
