Correlation among neutrophil/lymphocyte ratio,systemic immune inflammation index and contrast-induced nephropathy in elderly patients with acute coronary syndrome after primary percutaneous coronary intervention

Abstract:Objective To study the correlation among neutrophil/lymphocyte ratio(NLR),systemic immune inflammation index(SII)and contrast-induced nephropathy(CIN)in elderly patients with acute coronary syndrome(ACS)after primary percutaneous coronary intervention(pPCI).Methods Elderly ACS patients(n=231)undergone pPCI were chosen from People's Hospital of Lu`an City from Oct.2018 to Oct.2022.All patients were divided,according to whether CIN occurred or not,into CIN group(n=25)and non-CIN group(n=206).The baseline materials of the patients were retrospectively analyzed.The correlation among NLR,SII and CIN was analyzed by using Logistic regression analysis and ROC curve analysis.Results There were totally 25 patients with CIN(10.82%).The percentages of patients with hypertension,diabetes,ejection fraction<40%and glomerular filtration rate(GFR)<60%were significantly higher in CIN group than those in non-CIN group(P<0.05).The value of NLR and SII were significantly higher in CIN group than those in non-CIN group(P<0.05).The results of Logistic regression analysis showed that NLR(OR=1.234,95%CI:1.096~1.388,P<0.05)and SII(OR=1.001,95%CI:1.000~1.001,P<0.05)were closely correlated to CIN occurrence.The results of ROC curve analysis showed that AUC of NLR combined with SII in predicting CIN was 0.738(95%CI:0.632~0.845),Jordan index was 0.443,sensitivity was 0.720 and specificity was 0.723.Conclusion The levels of NLR and SII are significantly correlated to CIN occurrence in elderly ACS patient after pPCI,which can be taken as reliable marks for predicting CIN occurrence.
