Curative effect of recombinant human brain natriuretic peptide combined with dopamine on right heart failure induced by precapillary pulmonary arterial hypertension

Abstract：Objective To observe the clinical efficacy of recombinant human brain natriuretic peptide(rhBNP)combined with dopamine on right heart failure induced by precapillary pulmonary arterial hypertension(PAH).Methods The patients with right heart failure induced by precapillary PAH(n=104)were chosen from the First Hospital of Qinhuangdao City from Feb.2022 to Dec.2022.All patients were randomly divided into control group(given routine treatment,n=34),group A(given routine treatment and continuous intravenous infusion of rhBNP,n=33)and group B(given routine treatment and continuous intravenous infusions of rhBNP and dopamine,n=37).The levels of BNP,fractional area change(FAC)of right ventricle,tricuspid annular plane systolic excursion(TAPSE),right ventricular cardiac index(RCI)and pulmonary arterial mean pressure(PAMP),and incidence of hypotension during treatment were compared among all groups.Results The comparison in baseline data had no statistical difference among 3 groups.BNP decreased in 3 groups after treatment,and it was lower in groups A and B than that in control group(P<0.05)and lower in group B than that in group A(P<0.05).There was statistical difference among 3 groups(F=41.2,P<0.01).FAC,TAPSE and RCI increased in 3 groups after treatment,and they were higher in groups A and B than those in control group(P<0.05),and higher in group B than that in group A(P<0.05).There was statistical difference among 3 groups(FFAC=41.6,FTAPSE=48.0,FRCI=183.1,all P<0.01).PAMP decreased in 3 groups after treatment,and was lower in groups A and B than that in control group(P<0.05)and there was no statistical difference between group A and group B(P>0.05).There was statistical difference among 3 groups(F=5.0,P<0.01).The incidence rate of hypotension was 11.8%in control group,24.2%in group A and 2.7%in group B(P=0.023).Conclusion RhBNP combined with dopamine can effectively improve right heart function in patients with right heart failure induced by precapillary PAH and reduce the incidence of hypotension.
