Risk factors of dilated cardiomyopathy complicated by pulmonary arterial hypertension and their influence on prognosis

Abstract:Objective To discuss the risk factors of dilated cardiomyopathy(DCM)complicated by pulmonary arterial hypertension(PAH)and their influence on prognosis.Methods DCM patients(n=100)were chosen from Municipal Hospital of Qingdao City from Jan.2020 to Apr.2022,and all patients were divided,according whether they were complicated by PAH or not,into PAH group(n=34)and non-PAH group(n=66).The baseline materials were compared between 2 groups.The risk factors of DCM complicated by PAH were analyzed by using multi-factor Logistic regression analysis,and incidence rates of major adverse cardiovascular events(MACE)were recorded after 1-y followed-up.Results The number of patients with grades Ⅲ to Ⅳ of NYHA classification was significantly higher,and QRS wave of electrocardiogram,N-terminal pro-brain natriuretic peptide(NT-proBNP),albumin(ALB),left atrial diameter(LAD)and pulmonary artery systolic pressure(PASP)were significantly higher in PAH group than those in non-PAH group(P<0.05).The results of multi-factor Logistic regression analysis showed that QRS wave of electrocardiogram,NT-proBNP,ALB and LAD were risk factors of PAF in DCM patients(P<0.05).The patients were followed up to Apr.30,2023,there were 2 cases missed in PAH group and 3 missed in non-PAH group.The incidence rates of MACE were significantly higher in PAH group than those in non-PAH group(P<0.05).Conclusion The risk factors of PAF in DCM patients include QRS wave of electrocardiogram,NT-proBNP,ALB and LAD,and DCM complicated by PAH can promote incidence rates of MACE,which should be paid more attention to in clinical practice.
