Relationship between obesity and blood pressure variability and left atrial enlargement in patients with hypertension

Abstract:Objective Explore the correlation between body mass index(BMI),blood pressure variability,and left atrial diameter index in hypertensive patients.Method 167 hypertensive patients who visited the Department of Cardiology at the Second Affiliated Hospital of Zhengzhou University from June 2018 to June 2022 were selected.The patients were divided into a regular weight group(BMI:18.5~23.9 kg/m2),super weight group(BMI:24~27.9 kg/m2),and an obesity group(≥28 kg/m2)according to their BMI,The ambulatory blood pressure and cardiac color Doppler parameters of the patients in the three groups were compared and analyzed.Results Compared to the regular weight group and the overweight group,the obese group had more significant the standard deviation of 24 h diastolic blood pressure(24h DPBSD),the standard deviation of daytime systolic blood pressure(dSBPSD),the standard deviation of daytime diastolic blood pressure(dDBPSD),the standard deviation of nighttime systolic blood pressure(nSBPSD),and the standard deviation of nighttime diastolic blood pressure(nDBPSD),(P<0.05).Correlation analysis showed a positive correlation between BMI and left atrial diameter index(P<0.05,r=0.358).Correlation analysis showed a positive correlation between left atrial diameter index and 24-hour SPBSD,24-hour DPBSD,dSBPSD,nSBPSD,nDBPSD in the regular weight group(r=0.297,0.394,0.309,0.360,0.424);Left atrial diameter index was positively correlated with 24h SPBSD,24h DPBSD,dSBPSD,dDBPSD,nSBPSD,and nDBPSD in the overweight group(r=0.632,0.399,0.601,0.377,0.410,0.404).The left atrial diameter index in the obese group was positively correlated with 24h DPBSD,24h DPBSD,dSBPSD,dDBPSD,nSBPSD,and nDBPSD(r=0.747,0.556,0.699,0.430,0.781,0.722).Further multiple linear regression analysis showed that the left atrial diameter index was positively correlated with BMI and dSBPSD(P<0.05).Conclusion Blood pressure variability and BMI are positively associated with left atrial diameter index,indicating that obesity and increased blood pressure variability can lead to increased left atrial diameter in hypertensive patients.
