Predictive value of blood pressure variability and red blood cell distribution width to early renal impairment in patients with H-type hypertension

Abstract:Objective To investigate the predictive value of blood pressure variability(BPV)and red blood cell distribution width(RDW)to early renal impairment(ERI)in patients with H-type hypertension.Methods The patients with H-type hypertension(n=100)were chosen from Suzhou Municipal Hospital from Mar.2021 to Mar.2023,and divided,according to whether with ERI or not,into ERI group(n=36)and non-ERI group(n=64).The indexes of kidney function,24h systolic blood pressure variability(24h SBPV),24h diastolic blood pressure variability(24h DBPV)and RDW were compared between 2 groups.The correlation among BPV,RDW and indexes of kidney function,and their predictive value to ERI were analyzed.Results The levels of serum cystatin C(CysC)and urea albumin creatinine ratio(UACR)were significantly higher in ERI group than those in non-ERI group(P<0.05).The levels of 24h SBPV,24h DBPV and RDW were significantly higher in ERI group than those in non-ERI group(P<0.05).The results of Pearson correlation analysis showed that 24h SBPV,24h DBPV and RDW were positively correlated to CysC and UACR(P<0.05).The results of ROC curve analysis showed that in predicting ERI,AUC of 24h SBPV was 0.934(95%CI:0.883~0.985),AUC of 24h DBPV was 0.933(95%CI:0.882~0.985)and AUC of RDW was 0.973(95%CI:0.936~1.000).Conclusion The levels of 24h SBPV,24h DBPV and RDW increase significantly and they have higher predictive value to ERI in patients with H-type hypertension complicated by ERI.
