Correlation analysis of patent foramen ovale with migraine and efficacy evaluation of interventional closure

Abstract:Objective To investigate the incidence and risk factors of migraine in patent foramen ovale patients and to evaluate the efficacy of interventional occlusion in patients with migraine combined with PFO.Methods 288 PFO patients were enrolled in the Second Hospital of Lanzhou University and divided into a migraine group(n=93)and a non-migraine group(n=195)according to whether they combined with migraine.The two groups'general clinical data,laboratory,and imaging indicators were analyzed to explore the risk factors of migraine in PFO patients.At the same time,the frequency,duration,headache severity,and HIT-6 score of headache attacks in patients with PFO complicated with migraine before and half a year after operation were compared to evaluate the efficacy of interventional occlusion PFO patients with migraine.Results Of the 288 PFO patients,93(32.3%)had migraines of varying degrees.Compared with the non-migraine group,the migraine group was younger,had a higher proportion of female and medium PFO,and had more elevated platelet and homocysteine levels,with statistical significance(P<0.05).In the balance of stroke,coronary heart disease,diabetes and levels of BMI,systolic blood pressure,diastolic blood pressure,pulse pressure,mean arterial pressure,serum creatinine,fasting blood sugar,uric acid,cholesterol,triglycerides,high-density lipoprotein cholesterol,low-density lipoprotein cholesterol,hemoglobin,red blood cells,D-Dimer,RLS shunt volume,there was no significant difference between the two groups(P>0.05).Multivariate Logistic regression analysis showed that the incidence of migraine in PFO patients was related to platelet level and PFO aperture size.The higher the platelet level was,the higher the risk of migraine was(OR=1.005,95%CI:1.000~1.010,P=0.044).Compared with a small PFO size,a large PFO increased the risk of migraine(OR=2.722,95%CI:1.052~7.045,P=0.039).The frequency,duration,headache severity,and HIT-6 score of headache attacks in patients with PFO complicated with migraine after interventional occlusion for half a year were significantly lower than before surgery,with statistical significance(P<0.05).Conclusion The incidence of migraine in PFO patients is related to platelet level and PFO aperture size.Higher platelet levels and large PFOs increase the risk of migraine.Interventional occlusion in patients with PFO and migraine can effectively relieve migraine attacks.
