Clinical study of correlation between cardiac function and thyroid function in elderly patients with heart failure

Abstract:Objective To explore the correlation between cardiac function and thyroid function in elderly patients with heart failure.Methods From January 2022 to January 2023,121 male heart failure patients over 75 years old who had a history of coronary heart disease and met the New York heart function grading criteria were selected and divided into hypothyroidism group,subclinical hypothyroidism group,ESS(euthyroid sick syndrome)group and control group according to the diagnostic criteria.Analyze the number of cases and percentages of New York Heart Failure Classification and the levels of N-terminal pro-b-type natriuretic peptide(NT-proBNP)and B-type natriuretic peptide(BNP)in each group of patients.According to the Left Ventricular Ejection Fraction(LVEF)heart failure classification,patients were divided into groups of heart failure with reduced ejection fraction(HFrEF),heart failure with mildly reduced ejection fraction(HFmrEF),heart failure with preserved ejection fraction(HFpEF),and a control group with normal heart function.The thyroid hormone levels in each group of patients were studied.Analyze the correlation between age,heart failure-related indicators,and thyroid function respectively.Results The greater the degree of hypothyroidism,the higher the New York heart failure grade,and the higher the value of NT-proBNP and BNP,such as the highest percentage of patients with New York heart failure grade Ⅳ in hypothyroidism group was 41.94%(P<0.01);TSH in HFrEF group was significantly more elevated than that in LVEF grade control group,while TT4,TT3,FT4 and FT3 were lower than those in control group(P<0.01).TSH values in HFrEF,HFmrEF,and HFpEF groups were gradually increased,while TT4 and FT4 were gradually decreased(P<0.05).The age of enrolled patients(r=0.162,P<0.05),BNP(r=0.295,P<0.05),NT-proBNP(r=0.311,P<0.05),LVEF(r=0.513,P<0.05)levels were positively correlated with TSH levels,NT-proBNP was negatively correlated with FT4(r=-0.291,P<0.05),and LVEF was positively correlated with FT3(r=0.181,P<0.05).Conclusion Heart function is related to thyroid function in elderly patients with heart failure,and the degree of heart failure is more severe in patients with hypothyroidism.
