Risk factors related to worsening renal function after acute heart failure

Abstract:Objective Worsening renal function(WRF)is an important factor affecting the prognosis of acute heart failure(AHF),AHF combined with WRF significantly increases the difficulty of AHF treatment and affects the prognosis of AHF.To analyze the incidence and risk factors of worsening renal function(WRF),so as to lay a foundation for accurately predicting WRF incidence and improving the prognosis of acute heart failure(AHF).Methods A retrospective cohort study was used to analyze the clinical data in AHF patients(n=872)admitted to Chinese PLA General Hospital from Jan.2009 to Jan.2019.The patients'general information,and parameters of vital signs and important organ function were collected at admission time.Take the difference between the maximum value of serum creatinine(SCr)within 7 d after admission and the baseline SCr value at admission≥26.5 umol/L as the criteria for WRF,and the patients were divided into WRF group and non-WRF group.Firstly,the correlation between included indexes and WRF was given a single-factor Logistic regression analysis,and then the factors with P<0.05 and OR≠1 in single-factor Logistic regression analysis were included in multi-factor binary Logistic regression analysis.Results There were totally 872 patients enrolled,and the incidence of WRF was 30.7%.The results of multi-factor binary Logistic regression analysis showed that in baseline materials advanced age(OR=1.010,95%CI:1.000~1.020,P=0.050),increased cardiac troponin T(cTnT,OR=1.457,95%CI:1.141~1.860,P=0.003),increased creatinine(Cr,OR=1.002,95%CI:1.000~1.004,P=0.020),elevated blood Cl-(OR=1.029,95%CI:1.003~1.055,P=0.025),diabetes history(OR=1.602,95%CI:1.164~2.203,P=0.004)were risk factors of WRF after AHF,and increased hematocrit value(HCT,OR=0.100,95%CI:0.013~0.796,P=0.027)was a protective factor of WRF after AHF.Conclusion The incidence of WRF was 30.7%in inpatients with AHF.The increased cTnT,Cr and blood Cl-,advanced age and diabetes history are independent risk factors of WRF,and increased HCT is a protective factor of WRF in AHF patients.
