Analysis of clinical characteristics and in-hospital lipid reduction intensity in young adults with acute myocardial infarction and unstable angina pectoris

Abstract:Objective To explore the difference in clinical characteristics,differential value of clinical indexes,and intensity of lipid-lowing therapy in young patients with unstable angina pectoris(UAP)and acute myocardial infarction(AMI).Methods A total of 467 UAP patients aged≤45 years and 168 AMI patients admitted to Fuwai Central China Cardiovascular Hospital from October 2019 to October 2021 were enrolled in this study.The general clinical data,laboratory examination,and in-hospital lipid reduction program were collected,and evaluate the Gensini score of the patients.ROC curve was used to analyze the sensitivity and specificity of some indexes in the clinical identification of UAP and AMI patients.Results The proportion of male patients,smoking history,previous statin treatment history,serum triglyceride(TG),low-density lipoprotein cholesterol(LDL-C),remnant lipoprotein cholesterol(RC),non-high density lipoprotein cholesterol(non-HDL-C),fasting blood glucose,Gensini score and the number of coronary artery lesions in AMI group were significantly higher than those in UAP group,but lower in hypertension history and high-density lipoprotein cholesterol(HDL-C)group.The difference was statistically significant(P<0.05).Multivariate logistic regression showed that male(OR=3.580),previous history of hypertension(OR=1.541),previous history of statin treatment(OR=1.816),higher LDL-C(OR=1.357)and blood glucose(OR=1.091)were more likely to be AMI,but the differential ability of these indexes was limited,and the area under the curve of the combination of five indexes was the highest(AUC=0.659,P<0.05).Between the two groups,85%of the patients belong to ultra-high-risk coronary heart disease group,and more than 95%of the patients receive moderate or high-intensity lipid-lowing therapy.Conclusion Among young ACS patients,male,previous history of non-hypertension,previous history of statin treatment,higher LDL-C and blood glucose are more likely to be AMI patients,and coronary artery lesions in AMI patients are more serious.More than 85%of young ACS patients belonged to ultra-high risk ASCVD population,and more than 95%receive moderate or high-intensity lipid-lowing therapy.
