The efficacy and safety of 2C3L combined with Marshall intravenous ethanol chemical ablation for long-term persistent atrial fibrillation

Abstract:Objective The objective of this study was to observe and investigate the effectiveness and safety of combining 2C3L with ethanol infusion of the vein of Marshall(VOM)for the ablation of long-term persistent atrial fibrillation(AF).Methods Twelve patients with long-term persistent AF were included in the study.The following procedures were performed:ethanol infusion of VOM,2C3L radiofrequency ablation(2C:two circumferential pulmonary vein isolation;3L:three ablation lines including mitral isthmus line,left atrial top line,and tricuspid isthmus line),and verification of pulmonary vein isolation and blockade of each ablation line after restoring sinus rhythm.Results The average duration of ethanol infusion of VOM was(41.8±13.2)minutes.Among the 12 patients,ethanol infusion of VOM failed in 2 cases,resulting in a success rate of 83.3%.All patients achieved bilateral conduction blockade of pulmonary vein isolation and each ablation line.Among the two patients who underwent the 2C3L procedure alone,one experienced recurrent left atrial flutter during the 1-year follow-up.Among the ten patients who underwent the combined 2C3L with ethanol infusion of VOM,one had a recurrence of paroxysmal atrial fibrillation at the 1-year follow-up.The success rate of the 2C3L procedure alone was 50%(1/2),while the success rate of the combined 2C3L with ethanol infusion of VOM was 90%(9/10).Conclusions The combination of 2C3L with ethanol infusion of VOM for the ablation of long-term persistent atrial fibrillation demonstrates a high success rate and safety.
