Ultrasound characteristics of coronary intravascular lesions in patients with premature coronary heart disease and its relationship with Lp-PLA2

Abstract:Objective To observe the relationship between plasma lipoprotein-associated phospholipase A2 (Lp-PLA2) level and intravascular ultrasound (IVUS) characteristics in patients with premature coronary heart disease, and to explore the significance of Lp-PLA2 detection in evaluating the degree of plaque inflammation in patients with premature coronary heart disease. Methods Seventy patients with early-onset coronary heart disease (early-onset coronary heart disease group) and 120 patients with late-onset coronary heart disease (late coronary heart disease group) were selected. Lp-PLA2, coronary angiography (CAG) and IVUS were used to evaluate the morphological characteristics of plaques by enzyme-linked immunosorbent assay (ELSIA). The relationship between Lp-PLA2 level, plaque characteristics of IVUS and Lp-PLA2 level and plaque composition was analyzed. Results The mean plasma Lp-PLA2 level in patients with early-onset coronary heart disease was significantly higher than that in patients with late-onset coronary heart disease (P<0.01). The IVUS plaque load in the early-onset coronary heart disease group and the late-onset coronary heart disease group were 58.0% and 43.2%, respectively, and the remodeling indices were (1.34±0.95) and (1.11±1.27), respectively. The fiber cap thickness was (0.65±0.18). Mm and (1.01±0.21 mm) were statistically different (P<0.05). Plasma Lp-PLA2 was positively correlated with plaque load (r=0.531, P=0.017) and positively correlated with fiber cap thickness (r=0.137, P=0.033). Conclusion Early-onset coronary heart disease is rich in soft plaques and thin fibrous cap plaques. Plasma Lp-PLA2 levels are associated with plaque vulnerability indicators identified by IVUS.
