Therapeutic effect of early combined application of torasemide, tolvaptan and lyophilized recombinant human brain natriuretic peptide on acute heart failure

Abstract:Objective To observe the efficacy and safety of early combined application of torasemide, tolvaptan and lyophilized recombinant human brain natriuretic peptide (rhBNP) in the treatment of acute heart failure (AHF). Methods A total of 123 patients with acute heart failure who were admitted to the First Hospital of Shanxi Medical University from December 2016 to December 2017 were enrolled, including 81 males and 42 females. According to the treatment, they were divided into four groups: torasemide group (n=30), rhBNP group (n=25), tolvaptan group (n=30), and triple drug group (n=38). All patients were given basic treatment of heart failure. The torasemide group was given torasemide 40 mg/d; the rhBNP group was given rhBNP at 0.01 μg/(kg·min) for 5 days and torasemide 20 mg/d; tolvaptan group was given tolvaptan tablets 15 mg / d, torasemide 20 mg / d; the triple drug group was given torasemide 20 mg/d, and the rhBNP treatment was continued to be pumped for 5 days at 0.01 μg/(kg·min), and tolvaptan tablets 15 mg/d; all four groups were treated continuously for 5 days. The changes of heart failure symptoms and blood routine, liver and kidney function, amino terminal pro-brain natriuretic peptide (NT-proBNP), electrolytes, net output (24 hours total intake - total output) were observed. Results After treatment, the cardiac function of the four groups was significantly improved. Compared with patients in the torasemide group, the symptoms of heart failure improved more significantly in the triple-drug group, net output increased, NT-proBNP, creatinine, cys-c levels decreased, and blood sodium levels increased, the difference was statistically significant ( P<0.05);hospitalization time decreased, the difference was also statistically significant (P<0.05). Conclusion The early combination therapy with torsemide, tolvaptan and rhBNP can help significantly improve the symptoms of AHF, increase serum sodium levels, prevent acute renal injury, shorten hospitalization days and improve prognosis.
