Therapeutic effect of vitamin D2 on cardiac autonomic neuropathy in type 2 diabetes

Abstract:Objective To investigate the clinical efficacy of vitamin D2 (VD2) in the treatment of diabetic cardiac autonomic neuropathy (DCAN). Methods 240 patients with DCAN were divided into control group (n=80), low-dose group (n=80) and high-dose group (n=80) according to the same gender, age difference less than 5 years, and disease path difference less than 3 years. Based on basic treatment, the control group was injected with 0.9 ml of 0.9% sodium chloride. The low-dose group and the high-dose group were intramuscularly injected with vitamin D2 (7.5 mg in the low-dose group and 15 mg in the high-dose group), once every 2 weeks for 1 year. Heart rate variability (HRV) index including standard deviation of normal to normal (SDNN), standard deviation of the average normal to normal (SDANN), root mean square of successive RR-interval differences (RMSSD), the proportion of interval differences of successive normal-beat to normal-beat intervals greater than 50 ms (PNN50), and 25(OH) D3 concentration before treatment, after treatment and after 1 year of treatment were observed and compared, as well as effectiveness of treatment. Results There were no significant differences in pre-treatment general data, heart rate variability (SDNN, SDANN, RMSSD, PNN50), 25(OH)D3, and VD deficiency rates among the three groups (P>0.05). Compared with before treatment, at the end of treatment and 1 year after treatment, the SDNN increased, SDANN decreased, RMSSD and PNN50 increased, 25(OH)D3 level increased, and VD deficiency rate decreased. The difference was statistically significant (P<0.05). PNN50 and 25(OH)D3 levels were decreased after 1 year of treatment in the low-dose group compared with the end of treatment, and the difference was statistically significant (P<0.05). Compared with before treatment, at the end of treatment and 1 year after treatment, the SDNN increased, SDANN decreased, RMSSD and PNN50 increased, 25(OH)D3 level increased, and VD deficiency rate decreased. The above differences were statistically significant (P<0.05). Compared with the end of treatment, SDNN, SDANN, RMSSD and PNN50 were decreased in the high-dose group after 1 year of treatment, and the difference was statistically significant (P<0.05). At the end of treatment and after 1 year of treatment, compared with the control group, the SDNN, SDANN, RMSSD and PNN50 increased, the 25(OH)D3 level increased, and the VD deficiency rate decreased in the low-dose group and the high-dose group. The difference was statistically significant (P<0.05). Among them, the change in the high-dose group was more obvious, compared with the low-dose group, the difference was statistically significant (P<0.05); the high-dose group at the end of treatment and after 1 year of treatment, the treatment of the less effective dose group and the control There were significant differences in the group, and the differences were statistically significant (P<0.05). Conclusions VD2 treatment of type 2 diabetes with DCAN is effective, can significantly improve the nutritional status and heart rate variability of patients with VD, and the effect is dose-related, high dose (15 mg) has a better effect.
