Correlation between lipoprotein-associated phospholipase A2 and contrast-induced nephropathy in patients with ST-segment elevation myocardial infarction

Abstract:Objective To investigate the correlation between preoperation level of serum lipoprotein-associated phospholipase A2 (Lp-PLA2) and contrast-induced nephropathy (CIN) in patients with ST-segment elevation myocardial infarction (STEMI) undergone primary percutaneous coronary intervention (PPCI). Methods STEMI patients (n=356) undergone PPCI were chosen from Department of Cardiovascular Medicine in Yuebei People’s Hospital of Guangdong Province from May 2017 to July 2018. The patients were divided, according to the quartile of serum Lp-PLA2 level, into 4 groups, including Q1 group (Lp-PLA2<367 ng/ml), Q2 group (Lp-PLA2 from 367 ng/ml to 409 ng/ml), Q3 group (Lp-PLA2 from 409 ng/ml to 445 ng/ml) and Q4 group (Lp-PLA2>445 ng/ml). And the patients also divided into CIN group and non-CIN group. The general clinical data, preoperation levels of serum Lp-PLA2 and serum creatinine (SCr), and postoperation incidence of CIN were collected and analyzed. Results Among 356 STEMI patients, 35 (9.8%) suffered from CIN. The preoperation level of Lp-PLA2 was significantly higher in CIN group than that in non-CIN group [(438.2±156.4) ng/ml vs. (366.5±90.5) ng/ml, P<0.05]. The incidence of CIN was 2.2% in Q1 group, 7.9% in Q2 group, 12.4% in Q3 group and 16.9% in Q4 group, and the incidence increased along with the increase of serum Lp-PLA2 level (P<0.01). The results of Logistic regression analysis showed that, after controlling other risk factors, preoperation serum Lp-PLA2 level was an independent risk factor of CIN (OR=8.569, 95%CI: 1.150~63.816, P<0.01). Conclusion The preoperation level of serum Lp-PLA2 is closely correlated to CIN, and is an independent risk factor of CIN in STEMI patients undergone PPCI. It can be taken as an index for predicting CIN in STEMI undergone PPCI.
