The relationship between QRS wave fragmentation and ventricular arrhythmia and left ventricular systolic function in patients with acute myocardial infarction

Abstract:Objective To study the relationship between QRS wave fragmentation and ventricular arrhythmia and left ventricular systolic function in patients with acute myocardial infarction. Methods 117 patients with acute myocardial infarction admitted to cardiovascular medicine department of Xinjiang Production and Construction Corps Seventh Division Hospital from March 2016 to March 2018 were studied. All patients were divided into group A (fragmentation QRS wave, 41 cases), group B (pathological Q wave, 40 cases) and group C (fragmentation QRS wave combined with pathological Q wave, 36 cases) according to the different patterns of ECG QRS wave. The incidence of ventricular arrhythmia, left ventricular ejection fraction, and left ventricular end-diastolic diameter were measured by dynamic electrocardiogram and echocardiography. The main adverse cardiac events during hospitalization were compared between the three groups. Results There was no significant difference in the incidence of ventricular arrhythmias of grade I, II and III among the three groups (all P>0.05). Group A and group C of Ⅳ grade A and ⅣB, Ⅴ incidence of ventricular arrhythmias were higher than group B (P<0.05). The level of left ventricular ejection fraction in group A and group C was significantly lower than the group B, while the level of left ventricular diameter at the end of diastolic was significantly higher (all P<0.05). There was no significant difference in the incidence rates of cardiogenic death, cardiogenic shock and acute left heart failure in group A, group B and group C (all P>0.05). Conclusion QRS wave fragmentation is closely related to ventricular arrhythmia and left ventricular systolic function in patients with acute myocardial infarction, which can provide reference for diagnosis and prognosis evaluation.
