Relationship between levels of plasma visfatin and MMP-9 and myocardial remodeling in patients with rheumatic heart valve disease

Abstract:Objective To study the relationship between levels of plasma visfatin and matrix metalloproteinase-9 (MMP-9) and myocardial remodeling in patients with rheumatic heart valve disease (RHVD). Methods RHVD patients (n=73, study group) and healthy controls (n=50, control group) were chosen from Department of Cardiologic Surgery in the First School of Clinical Medicine of Lanzhou University from Feb. 2016 to Feb. 2018. The levels of plasma visfatin and MMP-9 were respectively compared between 2 groups. Furthermore, the study group was divided, according to the results of preoperation echocardiogram, into group A (left ventricular end-diastolic anteroposterior diameter≥50 mm, n=40) and group B (left ventricular end-diastolic anteroposterior diameter<50 mm, n=33). The myocardial histological specimens were taken for HE and Mason staining during operation. The levels of plasma visfatin and MMP-9, cardiomyocyte diameter and level of myocardial collagen volume fraction (CVF) were compared in 2 groups, and the correlation among them was analyzed. Results The level of plasma visfatin was (43.12±8.34) μg/L and level of plasma MMP-9 was (24.05±7.18) μg/L in study group, and level of plasma visfatin was (24.68± 9.01) μg/L and level of plasma MMP-9 was (17.34±9.40) μg/L in control group (all P<0.05). The level of plasma visfatin was (46.11±6.37) μg/L and level of plasma MMP-9 was (26.80±7.22) μg/L in group A, and level of plasma visfatin was (39.78±8.24) μg/L and level of plasma MMP-9 was (2.11±6.98) μg/L in group B (all P<0.05). The cardiomyocyte diameter was (34.01±5.12) μm and myocardial CVF level was (14.21±0.07) % in group A, and cardiomyocyte diameter was (27.73±6.48) μm and myocardial CVF level was (12.56±0.05) % in group B (all P<0.05). The results of Pearson correlation analysis showed that levels of plasma visfatin and MMP-9 of RHVD were positively correlated to cardiomyocyte diameter and myocardial CVF level (all P<0.05). Conclusion The levels of plasma visfatin and MMP-9 have higher expressions and they are closely correlated to myocardial remodeling, which can be taken as the indexes for reviewing severity of myocardial remodeling in clinical practice.
