Relationship between fasting blood glucose fluctuation and severity of coronary artery disease in patients with type 2 diabetes mellitus

Abstract:Objective To investigate the relationship between fasting blood glucose fluctuation and severity of coronary artery disease in patients with T2DM. Methods The clinical data and coronary angiography results of T2DM patients who underwent coronary artery angiography from January 2017 to December 2017 in Changzhi People's Hospital were retrospectively analyzed. According to the results of coronary angiography, SYNTAX score was performed. According to the results of SYNTAX score, the patients were divided into non-coronary heart disease group, mild group, moderate group and severe group. Bivariate correlation analysis and hierarchical Logistic regression were used to analyze the correlation between fasting blood glucose and coronary artery SYNTAX score. Results (1) A total of 73 patients, 53 males and 20 females, aged 61.6 + 12.4 years, were enrolled. Among them, 18 cases (24.7%) were without coronary heart disease (SYNTAX=0), 21 cases (28.8%) were in mild group (SYNTAX<22), 17 cases (23.3%) were in moderate group (22≤SYNTAX≤32) and 17 cases (23.3%) were in severe group (SYNTAX>32). (2) There were significant differences in insulin, low density lipoprotein, high density lipoprotein, thyroid stimulating hormone, mean fasting blood glucose, standard deviation of fasting blood glucose, coefficient of variation of fasting blood glucose and HbA1c between groups (P<0.05), and SYNTAX score increased with the increase of standard deviation of fasting blood glucose and HbA1c. (3) The correlation analysis indicated that the time to duration ratio of insulin use to diabetes mellitus (r=0.355, P<0.05), low density lipoprotein level (r=0.636, P<0.01), average level of fasting blood glucose (r=0.732, P<0.01), standard deviation of fasting blood glucose (r=0.704, P<0.01), HbA1c (r=0.809, P<0.01) were correlated with the SYNTAX score of coronary artery. (4) Using SYNTAX score as dependent variable, smoking history, family history, high density lipoprotein, low density lipoprotein, insulin use, mean fasting blood glucose, standard deviation of fasting blood glucose and HbA1c as independent variables, grade Logsitic regression analysis showed that the average fasting blood glucose (OR=2.22, 95%CI: 1.35~3.66, P<0.01), standard deviation of fasting blood glucose (OR=2.43, 95%CI: 1.19~4.97, P=0.02), HbA1c (OR=3.69, 95%CI: 1.88~7.25, P<0.01) were risk factors for the severity of SYNTAX score. Conclusion There is a correlation between mean fasting blood glucose, standard deviation of fasting blood glucose and SYNTAX score of coronary artery in patients with type 2 diabetes mellitus, which is a risk factor for the severity of coronary artery disease.
