Relationship between serum cTnIAAb and left ventricular remodeling in patients with acute myocardial infarction

Abstract:Objective To investigate the expression level of serum anti-cardiac troponin I autoantibody (cTnIAAb) in patients with acute myocardial infarction and its relationship with left ventricular remodeling. Methods A total of 143 patient with acute myocardial infarction admitted to Anzhen hospital from January 2016 to May 2018 as subjects, which were divided into NYHA class Ⅱ (n=62), NYHA class Ⅲ (n=47) and NYHA classⅣ (n=34) according to the cardiac function classification criteria formulated by the New York Heart Association (NYHA). 40 healthy volunteers were randomly selected as control group. Enzyme-linked immunosorbent assay (ELISA) was used to detect the A450 nm value of serum cTnIAAb in each group at admission, and left ventricular remodeling indexes such as LVEDD, LVESD were examined by echocardiography, and LVEDV, LVESV, LVEF, LVMI were measured by M-mode ultrasound. Results The A450 nm value of serum cTnIAAb in patients with acute myocardial infarction of different NYHA grades was higher than that of the control group, and the higher the NYHA grade, the higher the A450 nm value of serum cTnIAAb, the difference was statistically significant (P<0.05). The levels of LVEDD, LVESD, LVEDV, LVESV and LVMI in patients with acute myocardial infarction with different NYHA class were higher than those in control group, and which showed an increasing trend with the higher NYHA class, while LVEF level in patients with acute myocardial infarction with different NYHA class was lower than that in control group, and which showed an increasing trend with the higher NYHA class, the difference was statistically significant (P<0.05). Pearson product moment correlation analysis showed that serum cTnIAAb was positively associated with LVEDD, LVESD, LVEDV, LVESV and LVMI (r=0.637, 0.598, 0.517, 0.562, 0.632; P<0.05), which was negatively associated with LVEF (r=-0.622; P<0.05). Conclusion Abnormal elevation of serum cTnIAAb is closely related to left ventricular remodeling in patients with acute myocardial infarction. Early detection of cTnIAAb may be an important index for evaluating the severity of acute myocardial infarction.
