Expression of IL-17 and IL-35 in peripheral blood of patients with acute coronary syndrome and its clinical significance

Abstract:Objective To explore the expression of interleukin-17 (IL-17) and interleukin-35 (IL-35) in peripheral blood of patients with acute coronary syndrome (ACS) and its clinical significance. Methods A total of 113 patients with ACS admitted to Xinzhou District Hospital of Wuhan Central Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology from October 2016 to October 2018 were divided into acute myocardial infarction group (AMI group, n=62) and unstable angina pectoris group (UAP group, n=51), and who also were divided into mild lesion group (Gensini score: 0~30 points, n=49) and moderate lesion group (Gensini score: 31~60 points, n=34) and severe lesion group (Gensini score: >60 points, n=30). At the same time, 50 patients with stable angina pectoris (SAP group) and 50 healthy volunteers (control group) were randomly selected. The levels of IL-17 and IL-35 in peripheral blood was detected by the enzyme-linked immunosorbent assay. Results The serum IL-17, NT-proBNP and cTnI levels in AMI group and UAP group was higher than those in SAP group and control group, while IL-35 and LVEF was lower than those in SAP group and control group, and the serum IL-17, NT-proBNP and cTnI levels in AMI group was higher than those in UAP group, IL-35, LVEF was lower than those in UAP (P<0.05). With the severity of coronary artery lesions, the levels of serum IL-17, cTnI, NT-proBNP gradually increased and IL-35, LVEF gradually decreased (P<0.05). Serum IL-17 was positively associated with cTnI, NT-proBNP, Gensini scores (r=0.622, 0.573, 0.736, P<0.05) was negatively associated with LVEF (r=-0.689, P<0.05). Serum IL-35 was negatively associated with cTnI, NT-proBNP, Gensini scores (r=-0.671, -0.602, -0.729, P<0.05) and positively associated with LVEF (r=0.714, P<0.05). The AUC of IL-17 was 0.808, the best cut-off value was 37.81 pg/ml, the sensitivity and specificity was 0.73 and 0.77, respectively, and the accuracy was 0.72. The AUC of IL-35 was 0.813, the best cut-off value was 40.05 pg/ml, the sensitivity and specificity was 0.76 and 0.80, respectively, and the accuracy was 0.75. The AUC of IL-17 combined with IL-35 was 0.901, the sensitivity and specificity was 0.92 and 0.89, respectively, and the accuracy was 0.90. Conclusion Peripheral blood IL-17 and IL-35 is closely related to ACS types and severity of coronary artery lesions. Early detection can be used as markers for differential diagnosis of ACS and evaluation of severity of coronary artery lesions.
