Efficacy and economic evaluation of irbesartan or telmisartan combined with antihypertensive Chinese patent medicines

Abstract:Objective To compare the efficacy and economics of antihypertensive Chinese patent medicine combined with irbesartan or telmisartan, and to provide reference for the selection and related decision-making of clinical treatment options for clinical antihypertensive Chinese medicine. Methods We searched the CNKI, VIP, Wanfang, Embase, Pubmed, and Cochrane Library databases for randomized controlled clinical trials of Chinese patent medicines combined with irbesartan or telmisartan for the use of corresponding Western medicines for hypertension. Based on the perspective of the health care provider, the relative cost of indirect comparison and the cost of drugs in hypertension outpatient treatment were evaluated economically using the incremental cost-effectiveness ratio (ICER). Results 8 literatures were included in our study (n=1028), and the results show that the effective rate of irbesartan alone group and Songling Xuemaikang capsule/Qinggan Jiangya capsule/Tianma Gouteng granules combined with irbesartan group were 75.00%, 128.25% and 84.75%, respectively; and their costs were 353.13, 679.4 and 1037.53 yuan, respectively. The effective rate of telmisartan alone group and Songling Xuemaikang capsule/Yangyin Jiangya capsule combined with telmisartan group were 75.00%, 128.25% and 84.75%, respectively; and their costs were 353.13, 679.4 and 1037.53 yuan, respectively. When we used irbesartan as an intermediate drug, the ICER values of Songling Xuemaikang,Qinggan Jiangya Capsule, and Tianma Gouteng Granules, Capsules combined with Irbesartan were 3664.52, 14372.22, and 6390.93, respectively. Taking telmisartan as an intermediate drug, the ICER values of Songling Xuemaikang capsule and Yangyin Jiangya capsule combined with telmisartan were 612.71 and 7019.52, respectively. The univariate sensitivity analysis results were stable. Conclusion The combination of irbesartan and Tianma Gouteng granules was more effective than other Chinese patent medicines; when combined with telmisartan, Songling Xuemaikang capsules was more effective than Yangyin Jiangya capsules. For economic evaluation, whether it is combined with telmisartan or irbesartan, Songling Xuemaikang Capsule has economic advantages over other proprietary Chinese medicines.
