Application of D-dimer in diagnosis of acute myocardial infarction

Abstract:Objective To investigate the diagnostic value of D-dimer (DD) level in patients with acute myocardial infarction (AMI). Methods The patients with acute coronary syndrome (ACS , n=339) were chosen from Affiliated Hospital of North China University of Science and Technology from Jan. 2017 to Mar. 2018, and divided into group of unstable angina pectoris (UAP group, n=96) and AMI group (n=243). AMI group was divided, according to symptoms and ECG results, into group of ST-segment elevation myocardial infarction (STEMI group, n=132) and group of non-ST-segment elevation myocardial infarction (non-STEMI group, n=111). The level of DD was compared among all groups, and value of DD in AMI diagnosis was reviewed by using curve of ROC curve. Results The level of DD was higher in NSTEMI group (723.47±425.72) ng/ml than that in UAP group (432.82 ±268.19) ng/ml, and was higher in STEMI group (1021.73±604.54) ng/ml than that in UAP group and NSTEMI group (P<0.05). The AUC of ROC curve was 0.859 and the best cur-off value was 531.51 ng/mL in AMI diagnosis by DD. When DD level>531.51 ng/mL, its sensitivity in AMI diagnosis was 82.3%, specificity was 82.3%, positive predictive value was 92.2%, positive likelihood ratio was 11.8, negative predictive value was 64.8% and negative likelihood ratio was 0.54. Conclusion The increase of DD level has some diagnostic value in AMI patients.
