Clinical analysis of target organ damage in patients with hypertension by dynamic blood pressure monitoring

Abstract:The relationship between hypertension and target organ damage (TOD) was studied using 24-hour ambulatory blood pressure monitoring (ABPM). Methods Dynamic blood pressure monitoring data of outpatients and inpatients in the Rehabilitation Medicine Department of Beijing Sijiqing Hospital from June 2014 to June 2016 were collected. 24-hour ABPM was performed on 232 hypertensive patients. Grouping and statistical analysis were performed according to the number of target organ lesions. Results The number of target organ damage was closely related to 24-hour mean systolic blood pressure, blood pressure load, increased nighttime average diastolic blood pressure, and nocturnal abnormal rhythm of blood pressure. Patients with severe target organ damage were often accompanied by abnormal carotid intima thickness, cardiac hypertrophy and cardiovascular events (P<0.01), abnormal blood pressure nocturnal rhythm and high incidence of morning peak blood pressure (P<0.01). Conclusion TOD is closely related to 24-hour mean systolic blood pressure, blood pressure load, and nocturnal mean diastolic blood pressure. Blood pressure nighttime abnormal rhythm and morning peak blood pressure are more harmful. ABPM can help to detect nocturnal blood pressure abnormalities and morning peak blood pressure to guide clinical treatment to reduce TOD.
