Related factors and predictability of high sensitivity C-reactive protein to perioperative cardiovascular events of diabetes

Abstract:Objective To analyze the related factors to perioperative cardiovascular events (PCE) of diabetes, and discuss the value of level of high sensitivity C-reactive protein (hs-CRP) in veracity of PCE incidence prediction.Methods The diabetic patients (n=350) received non-cardiac operation were chosen from People's Hospital of Baoji City from Oct.2016 to Jan.2018, and divided, according to level of plasma hs-CRP, into lowdose group (hs-CRP<1.0 mg/L, n=50), mid-dose group (hs-CRP 1.0~3.0 mg/L, n=75) and high-dose group (hs-CRP>3.0mg/L, n=225).The correlation between single variable risk factor and PCE was analyzed by using Spearman rank correlation analysis.The risk factors related to PCE were analyzed in patients with P<0.05 by using binary Logistic regression analysis, and veracity of risk factors in prediction of PCE was analyzed by using ROC curve in diabetic patients.Results The incidence of PCE was 0 in low-dose group.There were 10 cases of severe arrhythmia (13.33%) and 12 cases of myocardial ischemia (16%) in mid-dose group.There were 32 cases of severe arrhythmia (14.22%) and 18 cases of myocardial ischemia (8.07%) in high-dose group.The results of Logistic regression analysis showed that grades of heart failure, age, level of glycated hemoglobin (HbAlc), echocardiogram and hs-CRP were independent risk factors of PCE in diabetic patients (P<0.05).The ROC of PCE in grades of heart failure was 0.385, 0.354 in HbAIc level, 0.819 in hs-CRP and 0.788 in echocardiogram.Conclusion The perioperative level of hs-CRP has some value in prediction of PCE in diabetic patients.
