Admission hyperglycemia and adverse outcomes in diabetic and non-diabetic patients with non-ST-elevation myocardial infarction undergoing percutaneous coronary intervention

Abstract:Objective This study evaluated the prognostic utility of admission hyperglycemia for 3-year major adverse cardiac events (MACEs) in Non-ST-elevation myocardial infarction (NSTEMI) patients undergoing percutaneous coronary intervention (PCI).Methods This study was a multicenter observation cohort study, and the study had been registered in the Chinese Clinical Trial Register, registration number is ChiCTR-PRN-15007233.Patients with confirmed NSTEMI undergoing percutaneous coronary intervention who were hospitalized between January 2009 and October 2012 were divided into diabetes patients'group and non-diabetes patients'group.According to the blood glucose concentration at admission, patients without diabetes were divided into two groups:admission blood glucose<144 mg/dl or≥144 mg/dl;patients with diabetes were divided into two groups:admission blood glucose<180 mg/dl or≥180 mg/dl.All patients were followed up for 3 years after discharge, and the outcomes were assessed.Results In non-diabetic patients, the incidence of major adverse cardiovascular events was higher in the 3 years of admission in the blood glucose≥144 mg/dl group compared with the hospitalized blood glucose<144mg/dl group, the difference was statistically significant (P<0.05).Among the patients with diabetes, compared with the group of patients with blood glucose<180 mg/dL, the incidence of major adverse cardiovascular events in 3 years was higher in the blood glucose≥180 mg/dL group, the difference was statistically significant (P<0.05).Patients with non-diabetic and admitted to the blood glucose<144 mg/dl group had the lowest incidence of major adverse cardiovascular events for 3 years, while those with diabetes and admitted to the blood glucose≥180 mg/dl group had the highest 3 years of major adverse cardiovascular events.The rate was statistically significant (P<0.05).Conclusion Regardless of whether the patient has diabetes or not, admission blood glucose levels can be used as an independent predictor of long-term major adverse cardiovascular events in patients with NSTEMI.
