Predictive value of NT-proBNP combined with neutrophil to lymphocyte ratio on short-term prognosis after early PCI in patients with high-risk Non-ST segment elevation myocardial infarction

Abstract:Objective To explore the predictive value of NT-proBNP combined with neutrophil to lymphocyte ratio on short-term prognosis after early PCI in patients with high-risk acute Non-ST segment elevation myocardial infarction (NSTEMI).Methods A total of 114 patients with NSTEMI who underwent early PCI were enrolled in the study.The clinical dates of patients, including age, sex, smoking, history of hypertension, diabetes and lipid abnormality were collected.The levels of serum NT-proBNP and whole blood NLR were measured before early PCI.During the follow up period of one-year, major adverse cardiac events (MACE) were recorded at 7 days in-hospital, 30 days and one year after PCI.According to whether the patients occurred MACE, the patients were divided into MACE group and non-MACE group.The level of NT-proBNP and NRL before PCI were compared between the two groups.Correlation between Gensini score and the levels of NT-proBNP, NLR were tested by Pearson correlation analysis.Risk factors of MACE within 1 year after early PCI in STEMI patients were analyzed by logistic regression analysis.Predictive value of NT-proBNP and NRL for short-prognosis after early PCI were evaluate by receiver operator characteristic curve.Results Compared with non-MACE group, the levels of NT-proBNP, NRL were significantly higher at 7 days in-hospital, 30 days and 1 year after PCI in MACE group (P<0.05).Multivariate logistic regression analysis showed that diabetes mellitus, multiple vessel disease, FMC-to-Balloon time prolonged, NT-proBNP and NLR level increased were the independent risk factors of MACE.Correlation analysis showed that the level of serum NT-proBNP and NLR have a positive correlation with Gensini score respectively (P<0.05).ROC analysis indicated the area under the roc curve (AUC) of NT-proBNP, NLR, NT-proBNP and NLR were 0.789(95%CI:0.762~0.815), 0.776(95%CI:0.758~0.832), 0.902(95%CI:0.813~0.932), respectively.The sensitivity of NT-proBNP, NLR, NT-proBNP and NLR were 82.8%, 81.9%, 87.4%, respectively.The sensitivity of NT-proBNP, NLR, NT-proBNP and NLR were 83.6%, 82.5%, 88.3%, respectively.Conclusion The levels of NT-proBNP and NLR before early PCI has positive correlation with severity of coronary artery disease in patients with acute NSTEMI, have certain predictive value on short-term prognosis after PCI.Integrated application of NT-proBNP and NLR are better than single index in evaluating short-term prognosis.
