Changes and clinical significance of levels of serum VCAM-1 and hs-CRP in patients with coronary bifurcation lesions before and after treatment

Abstract:Objective To analyze the changes and clinical significance of levels of serum vascular adhesion molecule-1(VCAM-1) and high sensitivity C-reactive protein (hs-CRP) in patients with coronary bifurcation lesions before and after treatment.Methods The patients (n=83) with coronary bifurcation lesions diagnosed by coronary angiography (CAG) were chosen as study group, and volunteers (n=50) with coronary artery stenosis below 70%were chosen as control group from Department of Cardiology of the First Hospital of Zhangjiakou City from Apr.2016 to Dec.2017.The study group was given Crush technique intervention and then continuously given tirofiban based on double anticoagulant therapy.The study group was divided, according to whether or not coronary artery restenosis occurred during the first year after the operation, into restenosis group (n=20) and normal group (n=63).The levels of VCAM-1and hs-CRP were detected in control group on the 2nd d, and in study group before, 3 d and 1 y after the operation, and at the same time heart rate (HR), mean arterial pressure (MAP) and cardiac output (CO) were detected.Results The levels of VCAM-1 hs-CRP and HR increased significantly, and levels of MAP and CO decreases significantly in study group compared with control group (P<0.05) before the operation.The levels of VCAM-1 hs-CRP and HR decreased significantly, and levels of MAP and CO increases significantly in study group 3 d and 1 y after the operation compared with before (P<0.05).The levels of VCAM-1, hs-CRP and HR were higher, and levels of MAP and CO were lower in restenosis group than those in normal group before and 3 d and 1 y after the operation (P<0.05).The results of Spearman correlation analysis showed that preoperative levels of VCAM-1 and hs-CRP were positively correlated to restenosis and HR (P<0.05), and negatively correlated to levels of MAP and CO (P<0.05).Conclusion The changes of levels of VCAM-1 and hs-CRP are correlated to postoperative restenosis and hemodynamics in patients with coronary bifurcation lesions after stenting intervention and anti-platelet treatment, which can be taken as indexes for reviewing treatment and prognosis of coronary bifurcation lesions.
