Relationship between neutrophil-to-lymphocyte ratio and severity of coronary artery lesions in young patients with coronary heart disease

Abstract:Objective To study the relationship between neutrophil-to-lymphocyte ratio (NLR) and severity of coronary artery lesions in young patients with coronary heart disease (CHD), and discuss the predictive effect of NLR on severity of coronary artery lesions. Methods The young patients with CHD (CHD group, n=134) and yang patients without CHD (non-CHD group, n=51) were chosen from Department of Cardiovascular Medicine of Shengjing Hospital affiliated to China Medical University from Jan. 2015 to Apr. 2016, and their medical records and relative physical examinations were checked. The CHD group was divided, according Gensini scores, into 2 subgroups (mild-to-moderate CHD subgroup, GS=1-49, n=80) and severe CHD subgroup (GS>49, n=53). The independent risk factors of severe CHD and efficacy of NLR in predicting severity of coronary artery lesions were analyzed by using Logistic regression analysis. The optimal cut-point and diagnostic value of NLR in predicting severity of coronary artery lesions were analyzed by receiver operating characteristic (ROC) curve. Results In CHD group, age, heart rate (HR), white blood cell count (WBC), neutrophil count (NEUT), NLR, platelet, total cholesterol (TC), low-density lipoprotein-cholesterol (LDL-C), triglyceride (TG) and cardiac troponin I (cTnI) were all higher than those in non-CHD group (all P<0.05 ). In severe CHD subgroup, HR, WBC, NEUT, NLR and cTnI were all higher than those in mild-to-moderate CHD subgroup (P<0.05). NLR was an independent risk factor for predicting high Gensini scores, and ROC curve analysis verified that the optimal cut-point of NLR in predicting high Gensini scores was 3.02, specificity was 81.3% and sensitivity was 63%. Conclusion NLR is correlated to severity of coronary artery lesions in young patients with CHD. The increase of NLR is an independent risk factor for severe coronary atherosclerosis in young patients with CHD. The higher the NLR level, the heavier the severity of CHD. Especially NLR>3.02 indicates severe coronary atherosclerosis.
