Clinical significance of coronary artery lesions in children with quantitative KD of coronary artery diameter Z by echocardiography

Abstract:Objective To investigate the value of echocardiography in coronary artery diameter Z value of quantitative measurement of clinical significance of Kawasaki disease severity of coronary artery disease description. Methods 132 children diagnosed as Kawasaki disease in the pediatrics College of University of South China and Hunan Children's Hospital from December of 2015 to May of 2017 with complete follow-up data were enrolled. Acute phase echocardiography was collected, coronary artery diameter was measured, clinical manifestations were recorded, and Z values were calculated. The Z values were divided into non expansion group (Z< 2.5), small coronary aneurysm group (2.5<Z<5), large coronary aneurysm group (5≤Z≤10) and large coronary aneurysm group (Z>10). The gender, age, clinical manifestation, fever time, laboratory examination results, coronary artery diameter and Z value were compared. Result The number of days of fever in four groups were (7.76±2.10) days, (8.90±1.32) days, (10.31±1.56) days, (12.05±1.09) days, respectively. The duration of fever was the shortest in the non-dilatation group. The duration of fever was longer in the small coronary aneurysm group, the large coronary aneurysm group, and was longest in the giant coronary aneurysm group. The number of platelets increased with the Z value of the coronary artery, and the coronary artery diameter in the acute and recovery stages of the KD children was compared. The platelet count increases as the Z value of the coronary artery increases. There was no significant difference in the diameter of the coronary artery between the acute phase and the recovery phase in the non-dilated group (P>0.05), but the Z value decreased significantly in the recovery phase (P<0.05). The coronary artery diameter and Z value of the small coronary artery aneurysm group and the giant coronary aneurysm group were significantly lower in the recovery period (P<0.05). Conclusion The conversion of coronary artery diameter to Z value can objectively reflect the severity of coronary artery disease and the process of recovery. It has important clinical significance for clinical treatment management and prognosis evaluation in the recovery stage.
